What is a Pacemaker?

A pacemaker is a small device placed under the skin, usually below the collarbone, that sends electrical impulses to the heart to maintain an adequate heart rate. It consists of:

  • A pulse generator (battery and electronics)

  • One or more leads (wires) that deliver impulses to the heart chambers

Indications for Pacemaker

Common indications include:

  • Symptomatic sinus node dysfunction (sick sinus syndrome)

The Implantation Procedure

  1. Pacemaker implantation is usually done under local anaesthesia with mild sedation.

  2. A small incision is made below the collarbone.

  3. Leads are advanced through a vein into the heart under X-ray guidance.

  4. Leads are tested for stability and electrical parameters.

  5. The pulse generator is connected to the leads and placed in a pocket under the skin.

  6. The incision is closed and dressing applied.

The procedure typically takes 45–90 minutes.

Post-Procedure Care

  • Monitoring overnight or as advised by the cardiologist.

  • Arm movements on the side of implantation are restricted for a short time.

  • Wound care instructions are given, and stitches are removed after healing.

  • Device checks are scheduled periodically to assess battery life and function.

Long-Term Follow-Up

Pacemakers can last 5–12 years depending on usage and device type. Regular follow-up includes:

  • Device interrogation with programmer

  • Checking battery status and lead performance

  • Adjusting settings based on patient symptoms and heart rhythm
